MercyCare

Reconnect Reconnect is a free, confidential outreach support service aimed at preventing homelessness by intervening early with families and young people to stabilize their housing situation and improve their level of engagement with family/work/education/training/local community. The service provides support through individual and /or family counselling, mediation, information, education, advocacy and referral.

Community and Home Support (C&HS) This service assists the elderly or people with disability, who need minimal home support, in enabling them to live well at home. C&HS staff helps individuals to maintain and regain independence using a “Wellness Philosophy”. C&HS concentrates on what people ‘can do’ for themselves rather than what they ‘can’t do’.

Settlement Services This service supports permanent residents who have arrived in Australia during the last five years as humanitarian entrants or family stream migrants, with a focus on those participating in the Adult Migrant Education Program (AMEP) Perth. Our program provides case work, information, referral, advocacy workshops to assist with settlement issues.

Family Mental Health Support Services (FMHSS) Family mental health support service is a free, confidential, service offering support to children, young people and their families who are experiencing issues that are making them feel sad, upset or stressed. FMHSS provides early intervention support to assist vulnerable families with children and young people who are at risk of/affected by mental illness.

Amber Youth Wellness Services. It provides a free outreach service and offers counselling and other psychology expertise; youth workers to help with other life challenges; peer workers and group activities and access to a medical consultant for additional support.

The service is for young people 12-25 and their families in the Northern suburbs who experience moderate to severe mental health needs or are facing complex life challenges, or who maybe looking for help to function better, improve their mental wellness and relationships.
